Questionnaire

Filling in this questionnaire is just one way you can share your family story. You can also contact Felicity Marlowe, campaign coordinator, to arrange a phone interview, or simply write something yourself and send it to us. Contact her by email or phone 0411 593 607.

 

Please write as much as you like for each question, but try to keep each answer a maximum of around 200 words! Please write in first person, using “I” or “we” statements like writing a story.

 

We'd love to include an image of your family – perhaps a photo or maybe your child could draw a picture! Please email us jpg file or similar.

 

Your story will be edited for clarity and brevity. You can specify whether or not to use your whole name, first name only, photos or other identifying details in the consent and privacy section at the bottom of the questionnaire. Contact us for more information.
